Appointments at City Eye Centre are available Monday to Friday. Prior to your appointment, please ensure you have a current referral from your referring doctor or optometrist in order for you to claim your Medicare rebate.
A CURRENT REFERRAL is always required at the time of your consultation. You will not be able to claim your Medicare Rebate if you are seen without a valid referral. Please ensure the practice has received your referral prior to or on the day of your appointment or your appointment may need to be rescheduled. Referrals can be obtained from your general practitioner, optometrist or other medical specialist. Unless specified, referrals from your GPs and optometrists are current for 12 months while specialist referrals only remain current for 3 months.
Your Medicare card, health fund details, list of current medications, allergies, and any other relevant medical details are also required at the time of your consultation.
Please advise our staff if the referral is urgent. Your referring practitioner can also contact our friendly staff for advice. For urgent retinal referrals after office hours, please contact Dr Lawrence Lee on 0418 799 676.
As your eyes may be dilated at your consultation, please arrange transport as you may be unable to drive for at least 3-4 hours. We also advise that an accompanying person comes with you. Please bring a pair of sunglasses which will reduce problems with glare. If you are a DVA patient, please ring to confirm that transport has been booked for you.
